  • 23/hour Position Summary The Early Intervention & Prevention Coordinator supports Choice Health Network's Ending the HIV Epidemic (EHE) initiatives by providing HIV prevention, early intervention services, and rapid linkage to care.
This role focuses on engaging individuals who are newly diagnosed, out of care, or at high risk for HIV, ensuring timely connection to medical services, improving retention in care, and reducing barriers to treatment. The position works collaboratively across clinical, prevention, pharmacy, and case management teams to support the HIV care continuum. Core Responsibilities Early Intervention & Linkage to Care
  • Provide early intervention services including HIV counseling, referrals, and linkage to care
  • Facilitate rapid linkage to care (goal: within 72 hours) for newly diagnosed individuals
  • Re-engage individuals who are out of care through outreach and follow-up
  • Support rapid start access to HIV treatment and prevention services Prevention & Testing
  • Provide risk-reduction counseling and HIV prevention education
  • Manage PrEP navigation, including enrollment, adherence support, and retention Care Coordination
  • Coordinate services across clinical, pharmacy, prevention, and case management teams
  • Support treatment adherence and appointment follow-up
  • Assist clients with access to insurance, medication assistance, and supportive services Outreach & Community Engagement
  • Participate in community outreach events and testing initiatives
  • Support response to potential HIV clusters/outbreaks through targeted engagement Support Services & Barrier Reduction
  • Identify and address barriers to care, including transportation and social service needs
  • Connect clients to housing, food, behavioral health, and other support services Documentation & Reporting
  • Maintain accurate documentation in the electronic medical record (EMR)
  • Complete required Ryan White, EHE, and state reporting
  • Support quality improvement and grant compliance activities
  • Maintain timely and accurate documentation in the electronic medical record (EMR), CAREWare & RWISE plus any additional systems we know this position may need use Qualifications Education & Experience
  • Bachelor's degree in Public Health, Social Work, or related field OR
  • Minimum of 2 years of relevant experience in HIV prevention, case management, or community health Skills & Competencies
  • Strong interpersonal skills with ability to build trust with diverse populations
  • Ability to work independently and within a multidisciplinary team
  • Knowledge of or willingness to learn HIV prevention, PrEP, and care continuum
  • Strong organizational, communication, and documentation skills
  • Commitment to harm reduction and social justice frameworks
  • Ability to maintain HIPAA compliance and confidentiality Key Performance Expectations

  • Accurate and timely data entry and reporting compliance
