PHC Coordinator

Ace Primary Home care is currently seeking highly motivated individual that is responsible for managing the placement of attendants in clients' homes, addressing client concerns, and ensuring high-quality service delivery within the Personal Home Care (PHC) program. Two years of PHC experience as a coordinator a must. This role requires strong communication skills and the ability to maintain positive relationships with clients and referral sources while adhering to program regulations and quality assurance standards. Experience as a PHC Coordinator is a must. Responsibilities Recruiting and placing attendants in clients' homes. Addressing client concerns and inquiries. Communicating with clients, referral sources, and doctor's offices via phone and in writing. Performing data entry tasks related to client and program information. Problem-solving to address issues that arise within the program. Maintaining positive working relationships with referral sources. Providing outstanding customer service to clients and their families. Monitoring and tracking program authorizations. Ensuring timely completion of paperwork and compliance with program regulations. Requirements Two years of PHC experience as a coordinator a must. Excellent written and verbal communication skills. Positive attitude and ability to work independently. Compassion for working with the aged and disabled. Verifiable work history with good references.
