Collects and documents patient data, collaborates with the team to establish priorities and set realistic goals, provides individual and group diabetes education and lifestyle counseling, and evaluates patient outcomes related to diabetes management and education. Ensures program compliance with current accreditation standards and coordinates activities to maintain accreditation standards and program recognition set by the current accrediting body. Qualifications Education Required Bachelor's degree in health-care related field

Experience:

Required More than five years of healthcare experience and/ or education experience

Certification/Licensure:

Required Cert Diabetes Educator (CDE) certification Required within one year of start date Licensed Dietitian (RD/LD) or Registered Nurse (RN)
