Job Description

Looking for a part time QDDP to ensure service plans are compliant, assessments are approved, records are maintained, and coordination occurs across all stakeholders. Conduct, review, and approve assessments (Initial, Comprehensive, Annual) in line with DBHDS and DMAS standards. Develop, implement, monitor, and update Individualized Service Plans (ISPs/Part V) for both initial and annual reviews and revisions as necessary. Complete Quarterly Reports and Discharge Summaries. Participate in SIS (Supports Intensity Scale), Annual, Quarterly Service Reviews, and other required meetings. Review documentation, perform audits, and ensure compliance with DBHDS licensing (e.g. Ch. 105), DMAS waiver requirements, and Office of Human Rights. Coordinate with Support Coordinators, providers, and family/caregivers (in conjunction with the provider) to schedule meetings and ensure supports are delivered per plan. Maintain accurate records, ensuring HIPAA compliance and timely documentation.
Education:
Bachelor's degree in a human services field (e.g. social work, psychology, special education, rehabilitation counseling, sociology)
Experience:
At least one (1) year of documented experience working directly with individuals who have intellectual or developmental disabilities.
Licenses/Certifications:
Valid VA driver's license required.
Skills:
Strong written and oral communication. Strong documentation and record‐keeping. Familiarity with behavior support plans, positive behavioral supports. Knowledge of DBHDS licensing regulations, DMAS DD waiver regulations, Office of Human Rights, relevant state and federal laws. Ability to manage multiple tasks, work independently, and coordinate across teams. Preferred Attributes Experience with the Supports Intensity Scale (SIS). Experience in the completion of audits of service provider records, reviewing compliance. Strong organizational, time management, and interpersonal skills. Flexibility to travel across the Hampton Roads area.
Working Conditions:
Some travel required Periodic meetings may occur in person across multiple cities.
