Through California's Enhanced Care Management (ECM) program, we provide intensive care coordination for individuals and families with complex needs. To deliver exceptional support, Esteem Health has pioneered a two-person Lead Care Manager team model .
Each care team consists of:
Inbound Lead Care Manager (ILCM)
  • Headquarters-based care planning and coordination Outbound Lead Care Manager (OLCM)
  • Field-based outreach and member engagement Together, this team supports a shared caseload of approximately 100 members , combining structured care planning with strong in-person community support.
What We're Looking For We are seeking individuals who are passionate about helping underserved populations and who thrive in either a structured planning role or a dynamic field-based role . Qualifications Fluency in Spanish (required) & English Experience in case management, care coordination, community outreach, or a related field (preferred but not required) Familiarity with CalAIM Enhanced Care Management (ECM) is a plus Strong communication and interpersonal skills Ability to work both independently and collaboratively within a team Passion for serving vulnerable populations Strong organizational and documentation skills Additional strengths that help candidates succeed: Inbound candidates often excel at: Organization and systems Strategic planning Documentation and compliance Outbound candidates often excel at: Relationship building Field-based outreach Adaptability in dynamic environments The Two Roles on the
Care Team:
Inbound Lead Care Manager (ILCM) The Inbound Lead Care Manager serves as the anchor of the care team , working primarily from headquarters in a structured and organized environment. This role focuses on building and executing the strategic care plan ("flight plan") for each member through proactive planning, electronic communication, and documentation. Key Responsibilities Build the Daily "Flight Plan" : Develop and implement structured care plans for members. Coordinate scheduling, services, referrals, and program participation.
Electronic Member Engagement :
Serve as the primary electronic contact for members through phone, text, email, and digital platforms. Ensure completion of critical forms and documentation. Connect Members with
Resources:
Coordinate medical, behavioral health, and community-based services aligned with each member's care plan.
Proactive Planning :
Anticipate upcoming needs and ensure members have a clear path forward.
Documentation & Compliance :
Maintain detailed and accurate records in the electronic health record (EHR). Ensure all ECM program documentation requirements are met. Team Support Collaborate closely with the Outbound Lead Care Manager and ECM Team Lead. Provide support for the outbound lead care manager for urgent situations and field visits if necessary. Outbound Lead Care Manager (OLCM) The Outbound Lead Care Manager is the face of the care team , spending the majority of time in the community meeting members where they are. This role focuses on relationship building, outreach, and real-time problem solving during in-person visits.
Key Responsibilities Lead Member Connection :
Build trust and rapport through consistent in-person engagement with members.
Physical Outreach & Community Connection :
Conduct regular field visits throughout the community. Typically complete three in-person member interactions per day .
Address In-the-Moment Needs :
Identify and address immediate needs during visits. Ensure members have access to appropriate services and resources.
Advocacy & Support :
Advocate for members in their homes, communities, and service systems.
Team Collaboration :
Provide real-world context and updates to the Inbound Lead Care Manager. Help ensure the care plan reflects the member's lived experience.
