Job Description

We are looking for a compassionate and organized Case Manager to support formerly homeless adults in San Francisco, California through a Contract position. This role combines hands-on client support, community-based outreach, and administrative coordination, making it ideal for someone with strong computer skills, excellent customer service, and comfort working throughout the Tenderloin area. The successful candidate will help residents maintain stable housing, connect with essential services, and navigate daily challenges with dignity and consistency.
Responsibilities:
  • Oversee a high-volume portfolio of residents and provide ongoing case management focused on housing stability, wellness, and progress toward individual goals.
  • Meet with tenants regularly through move-in outreach, scheduled follow-ups, and as-needed visits to assess needs and adjust support plans.
  • Build trusting relationships with residents and use proactive engagement strategies to encourage participation, reduce barriers, and strengthen housing retention.
  • Help clients secure and maintain benefits, stay current with rent obligations, and resolve practical issues that may affect tenancy.
  • Coordinate with property staff during unit checks, pest control visits, and habitability concerns to support safe and stable living conditions.
  • Connect residents with community resources such as healthcare, behavioral health, substance use services, and employment programs, and follow up to encourage successful participation.
  • Organize tenant groups, social activities, and community events that promote connection, engagement, and a supportive residential environment.
  • Respond to urgent tenant situations with calm judgment, including de-escalation support, documentation, and communication with internal teams or outside providers when needed.
  • Maintain accurate case records, protect confidential information, and complete required documentation, reporting, and meeting participation in a timely manner.
