This position ensures quality delivery of the Housing Trio services (Housing Transition Navigation Services, Housing Tenancy and Sustaining Services, and Housing Deposits) and Transitional Rent benefits, while maintaining regulatory compliance, staff development, and program performance. The Supervisor coordinates with Managed Care Plans, Enhanced Care Management providers, behavioral health agencies, and community partners to achieve housing stability outcomes for members. The community Supports Supervisor ensures systematic coordination of services through a community-based, interdisciplinary, trauma informed, harm reduction and person-centered approach. Key Responsibilities Program Supervision Provide individual and group supervision to Community Supports Case Managers. Review and approve housing assessments, housing support plans, and service authorizations. Ensure adherence to trauma-informed care, Housing First, harm reduction, and person-centered practice principles. Provide consultation on complex cases involving multiple barriers to housing or requiring coordination across multiple systems. Quality Assurance and Compliance Monitor service delivery for compliance with City Net and DHCS Community Supports Policy Guide. Conduct case file reviews and documentation audits. Ensure housing settings meet HUD quality standards or state habitability requirements. Track quality metrics, housing outcomes, and service utilization data. Staff Development and Training Onboard and train new Case Managers on Community Supports services, housing navigation, landlord engagement, and tenant rights. Provide ongoing professional development on Fair Housing law, reasonable accommodations, Transitional Rent requirements, and coordination with BHSA Housing Interventions. Conduct performance evaluations and identify skill development needs. System Coordination Coordinate member referrals, service authorizations, and billing requirements. Facilitate partnerships with ECM providers, county behavioral health agencies, Coordinated Entry Systems, public housing authorities, landlords, and homeless service providers. Ensure effective referral pathways and warm hand-offs between housing services and other supports. Data Management and Reporting Monitor caseload ratios, productivity standards, and authorization timeframes. Participate in MCP monitoring activities and quality improvement initiatives. Landlord and Community Relations Build and maintain relationships with landlords, property managers, and affordable housing providers. Address landlord concerns and facilitate dispute resolution. Develop strategies to expand housing options and overcome discrimination. Represent the program in community coalitions and housing collaborative meetings. Team Leadership Provide leadership to build an effective, trauma-informed team. Manage scheduling, coverage, and resource allocation. Foster a culture of collaboration, cultural responsiveness, and member-centered practice. Support staff wellness and prevent burnout through appropriate supervision and workload management. Address staff concerns and facilitate conflict resolution. Direct Service Provision Maintain a limited caseload of members with complex housing barriers. Provide backup coverage for Case Managers as needed. Respond to escalated member or landlord concerns and housing crises. Model best practices in housing navigation, tenancy sustaining, and landlord engagement. Required Qualifications Minimum 3 years of experience in care management, case management, or care coordination for individuals with complex medical, behavioral health, and/or social needs Minimum 2 years of supervisory or leadership experience overseeing housing or case management staff Deep expertise in homelessness, housing barriers, trauma-informed care, and Housing First principles Comprehensive knowledge of HUD programs, permanent supportive housing, rental subsidies, tenant rights, landlord-tenant law, and Fair Housing regulations Understanding of Medi-Cal managed care, CalAIM Community Supports, and BHSA Housing Interventions Strong leadership, coaching, and performance management skills Excellent communication, interpersonal, and conflict resolution abilities with diverse stakeholders Proficiency with electronic health records, data analysis tools, and care management software Valid driver's license and reliable transportation for community-based oversight Eligibility to drive company vehicles. For insurance purposes, drivers of City Net vehicles must be at least 21 years old, have held a valid driver's license for at least four years, and must be able to pass a review of their motor vehicle report. Ability to pass background check and meet provider credentialing requirements Preferred Qualifications Master's degree in Social Work (MSW), Counseling, Public Administration, or related field LCSW, LPCC, LMFT, or Certified Housing Navigator credential Experience with CalAIM Community Supports program implementation and oversight Experience with Coordinated Entry Systems, HMIS data management, and housing outcome measurement Knowledge of behavioral health systems, substance use disorder treatment, and coordinating with county behavioral health agencies Bilingual proficiency in Spanish or other languages commonly spoken in the service area Lived experience with homelessness or housing instability Work Environment and Physical Requirements This position is based in an office setting with regular travel to housing sites, community locations, landlord meetings, and member homes for staff supervision and program oversight. Work may require occasional evening or weekend hours to support staff and member needs. Physical requirements include ability to drive, walk, climb stairs, and access various housing settings.
