- $78.72 an hour
- Full-Time Days, 40 hours per week Boulder Community Health is seeking an Epic Community Connect Program Manager responsible for the end-to-end performance, operational success, and continuous optimization of BCH's Community Connect program.

Master's degree in Informatics, MBA, Information Technology, or other Healthcare-related field preferred Project Management Professional certification preferred 3 years of experience in healthcare IT, clinical informatics, operations, or program/project management required. Five years preferred. Experience working with Epic or other EHR platforms strongly preferred Experience leading cross-functional initiatives in a healthcare setting preferred Responsibilities Own the overall performance and success of assigned Community Connect sites, including clinical, operational, and technical outcomes. Partner with BCH and site leadership to define priorities, establish measurable goals, and align initiatives with organizational strategy. Identify workflow, operational, and technology issues; conduct root cause analysis; and lead implementation of sustainable solutions and process improvements. Oversee intake, prioritization, and execution of break-fix, optimization, and project work in partnership with IT and Informatics teams. Monitor program performance metrics, drive continuous improvement, and support value-based clinical, operational, and financial outcomes. Lead cross-functional teams and influence stakeholders across clinical, operational, and IT environments to drive alignment, accountability, and decision-making. Serve as a trusted advisor to Connect site leaders by providing strategic guidance on Epic optimization, workflows, and operational best practices. Develop and manage project plans, communication strategies, risk mitigation plans, Statements of Work (SOWs), resource plans, and cost estimates. Coordinate multidisciplinary teams to deliver projects on time, within scope, and aligned with BCH governance and organizational standards. Evaluate requests for feasibility, level of effort, and cost impact to support prioritization and governance decisions.
