Job Description:
Supervisors are responsible for maintaining standards set by the State of Minnesota (Primarily Rule 245D), for the residential home for which they are assigned. This includes scheduling, team building, team management, writing plans and progress reviews, communicating regularly with teams, assist with life planning, maintaining compliance, attend meetings, training etc.
Job Responsibilities:
Supervise Direct Support Professionals Facilitate Person Centered Service Delivery Maintain regular Team Correspondence Quality Control on all program standards Maintain designated staffing patterns Promote independence and facilitate growth Provide leadership and support each individual with achieving their life's goals Offer mentorship and direction on problem solving

Associate's Degree Baccalaureate Degree Education and Experience Minimum Requirements:
Must qualify as a Designated Coordinator as defined by Minnesota Statute Chapter 245D; as defined in clauses (1-4. a) listed below: (1) A Baccalaureate degree in a field related to human services, education, or health, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or equivalent work experience providing care or education to vulnerable adults or children; (2) An Associate degree in a field related to human services, education, or health, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or equivalent work experience providing care or education to vulnerable adults or children; (3) A diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or equivalent work experience providing care or education to vulnerable adults or children;
OR:
(4) A minimum of 50 hours of education and training related to human services and disabilities and: ( 4.a) A minimum of four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1-3) as listed above.
