Job Description

Job Summary We are seeking a dedicated and compassionate Case Manager to join our dynamic residential care team. In this vital role, you will coordinate resident care plans, facilitate communication among multidisciplinary teams, and ensure seamless discharge planning and utilization management. Your expertise will help optimize resident outcomes, improve care efficiency, and support residents through the residential and aftercare process. This position offers an exciting opportunity to make a meaningful difference in the lives of children while working in a fast-paced, collaborative environment. Duties Assess resident needs through comprehensive intake and clinical evaluations. Develop individualized monthly treatment plans. Coordinate discharge planning with the referral agency at the time of admission. Conduct utilization review and management to ensure appropriate level of care. Collaborate with social workers, physicians, nurses, and other healthcare professionals to support patient service goals and manage complex cases involving managed care. Facilitate communication between residents, families, insurance providers, juvenile officers, GAL's, and teams to streamline discharge processes and ensure continuity of care. Participate in ongoing treatment planning with licensed therapist alongside social workers to work with the resident towards their discharge goal, Requirements Proven experience in a case management role or related field. A background in social work with emphasis on case management in a residential setting. Excellent communication skills to effectively collaborate across multidisciplinary teams including behavioral health specialists. A strong sense of organization Ability to multitask Critical thinking skills combined with a proactive approach to problem-solving in fast-paced residential settings. Experience working with children in a behavioral health setting is preferred.
