- For eligible positions About the Job To use best practices and technology within a community based residential setting to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible.
Requirements Minimum:
High School Diploma (or equivalent) with 1 year experience working with individuals with intellectual and/or developmental disabilities
Preferred:
Bachelor's degree with at least 1 year experience working with people with developmental disabilities, or high school diploma (or equivalent) with three years' experience working with individuals with intellectual and/or developmental disabilities including at least 1 year supervisory experience Valid unrestricted driver's license with good driving record Basic Computer Literacy Required Ability to lift and transfer at least 50 pounds The ability to sit, stand, bend, squat, reach, be agile, physically pursue individuals on foot and stand for several hours at a time Must be 18 years of age or older Must be able to read, write and verbally communicate in English and direct/respond to individual's needs Responsibilities Works a flexible 40 hour week schedule consistently and able to work extended hours to cover necessary shifts as and when needed. In conjunction with the Support Manager, ensures that the program staff complete all required documents of the individuals on the EHR correctly and timely for Medicaid billing. In conjunction with the support manager, monitors and insures accurate "in house" documentation. In conjunction with the support manager, reports, monitors and schedules house and vehicle maintenance. In conjunction with the support manager, assists in maintaining and monitoring of individual finances, as needed. In conjunction with the support manager, monitors and reports health and medical needs of individuals, and also audits the medication administration. Builds a support network between the individuals and community members. Documents person's progress for support people including family, staff, employment specialist, case manager and other professionals as necessary. In conjunction with the support manager, ensures consistent service delivery, which connects
- work home, and recreational needs.
Serves as an active member of each person's interdisciplinary team, to include involvement in Individual Service Plan (ISP) development . Encourages and ensures that individuals maintain a clean, safe, and comfortable environment providing assistance as appropriate. Attends conferences, seminars, and training relevant to the position and to the individuals served. Successfully completes required residential training including but not limited to: CPR and First Aid; Medication Administration; Preventing Abuse, Neglect and Exploitation; Shifting Expectations, Life Experience and Services; In-House Orientation; Crisis Management; New Hire Orientation; Life Threatening Emergencies (Danielle's Law); Stephen Komninos' Law; and Corporate Compliance. Works with each individual's treatment team, IDT and all other interested parties to build self-esteem and social capitol for each individual served. Successfully and consistently scores a 90% rating or higher on Quality Assurance audits. A rating of 89% or less for 3 consecutive reviews may result in immediate termination. Other duties as assigned. On call 24 hours a day in cooperation with other management/administrative/scheduling staff. Provides direct care, medication administration and assistance with daily living skills to the individuals receiving services appropriately and professionally with respect and dignity. In conjunction with the support manager, objectively completes staff evaluations. In conjunction with the support manager, monitors and maintains residential budget. In conjunction with the support manager, monitors and schedules required staff training, as well as relevant seminars, conferences and additional training. In conjunction with the Support Manager, ensures all program staff are trained in the Electronic Healthcare Record (EHR) system for Fee for Service in a timely manner.
