Responsibilities:

Supervise a multi-disciplinary team that provides intensive case management to young adults ages 18-24 who have experienced out of home placement and housing instability. Oversee day to day operations of implementing an evidenced-based program model. Effectively manage the electronic referral system. Meet with young adults eligible to apply for the program and be able to assess their readiness for program entry. Conduct community outreach/presentations in the community; develop and maintain community relationships with employers and post-secondary schools/programs to support young adults in the program. Provide after hours on-call coverage to direct staff on-call

Minimum Skills:

BA/BS in a social service field plus 5 years of supervisory experience; Master's degree in a related field preferred. Bilingual preferred. Ability to work independently and provide supervision to the team with strong organization skills, knowledge of community, and the ability to advocate for former foster youth. Acceptable driver's license history.
