Associates degree Clean and valid NYS Driver's License AMAP Certification (Paid Training Provided) Prior community experience with DD population

Responsibilities:

Identify, develop and coordinate sites within community for participants consistent with their needs, desires and valued outcomes. Coordinate planning of activities at sites with community coach as per individual's needs, desires and valued outcomes. Assess sites for wheelchair accessibility. Support active participation in the community and promote community connections. Ensure needs of individuals are met including but not limited to their safety and well-being. Drive individuals to and from community locations. Ensure compliance with

OPWDD/OPTS

regulations and Day Habilitation Program Policy & Procedure, to include but not limited to training on behavioral plans/strategies & protocols. Maintain daily log of services provided in the community. Provide coverage in program as needed. Provide participants with a community integration experience daily, with or without additional staff, etc. as appropriate. Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.

Physical Demands:

Must be able to lift, bend and push more than 100 lbs. Regularly required to stand and walk for extended periods. Regularly required to use hand-over-hand assistance when necessary. Occasionally exposed to moving mechanical parts and outside weather conditions. Required to drive during inclement weather to community sites. Join our Team and Make a Difference!

The Cerebral Palsy Association of Nassau County is a nonprofit, multi-faceted health agency serving over 1,500 children and adults with cerebral palsy, developmental and other disabilities. Day Habilitation is a year-round program serving individuals ages 21 and older who are developmentally disabled. Day Hab is a community outreach program where trained and qualified individuals with developmental disabilities provide local non•profit businesses with community service. It is a comprehensive program which focuses on community integration and volunteerism. This position ensures individuals with disabilities are fully integrated into the community through volunteering and/or participating in community activities. Integration Specialist is responsible for providing employment and/or community participation supports and services that develop and expand the individual's access to and participation in their preferred/desired community activities, while building relationships with community members, family, friends and neighbors.
