Job Description

Rapid Rehousing & Community Support Specialist
Location:
Troy Street, Dayton, Ohio
Schedule:
Monday-Friday, 8:00 AM - 5:00 PM Eastway Behavioral Healthcare is seeking a compassionate and dedicated Housing & Community Support Specialist to join our team. This role supports individuals experiencing homelessness in securing and maintaining safe, stable housing through a Housing First, trauma-informed approach. This role combines housing navigation with supportive service coordination, working collaboratively with clients, landlords, and community partners to remove barriers, promote self-sufficiency, and ensure long-term housing stability.
Key Responsibilities:
  • Serve as the primary point of contact for housing referrals, coordinate housing search appointments, and assist clients in locating and securing safe, affordable housing in compliance with HUD guidelines, FMR, and utility standards.
  • Build and maintain relationships with landlords, property managers, and housing providers; educate partners on program requirements and maintain a landlord database.
  • Coordinate all move-in logistics including inspections (NSPIRE), lease signings, utilities, and household setup; identify and address habitability concerns.
  • Conduct outreach and engagement with individuals experiencing homelessness to build rapport and connect them to services.
  • Collaborate with clients to identify and remove barriers to housing, including obtaining identification, income, benefits, and resolving rental or utility issues.
  • Develop, implement, and regularly update Individual Service Plans (ISPs) focused on housing stability, income, and activities of daily living using a Housing First, trauma-informed, and harm reduction approach.
  • Provide ongoing case management support, including home visits, service coordination, and referrals to community resources (e.g., JFS, Social Security, medical, substance use, and vocational services).
  • Support clients in obtaining and maintaining employment, education, and income to promote long-term self-sufficiency.
  • Advocate for clients and mediate landlord-tenant issues to prevent eviction and ensure housing stability.
  • Maintain accurate, timely, and compliant documentation in HMIS and agency systems, including service notes within 24 hours, ISP updates (at least every 30 days), discharge summaries, and 90-day eligibility recertifications.
  • Participate in data tracking, team meetings, case conferences, supervision, and required trainings; collaborate with internal and external partners.
  • Ensure effective communication by responding to inquiries within one business day and completing all assigned duties in a timely and professional manner.
Qualifications:
Minimum:
High School diploma with 3 years of relevant experience.
Preferred:
Bachelor's degree in Social Work, Mental Health, or a related field. Experience working with the homeless, mentally ill or SUD population. Experience with HMIS is a plus. Valid Ohio Driver's License, current car insurance required, and transportation as position requires the ability to transport clients.
