kgcdc.org Position Summary This dual role possesses a genuine commitment to the mission of the Kingdom Global Community Development Corporation (KGCDC). The position contributes to this mission through direct client care coordination and community health education, building relationships with diverse audiences in the Washington DC metropolitan area, with a primary focus on Prince George's and Montgomery counties in MD. What You'll Do Guide Clients Through Their Journey Facilitate comprehensive client intake processes, ensuring all relevant information is collected efficiently and accurately Engage with current and potential clients through various communication methods to answer questions and gather essential information about their unique situations Evaluate client needs with compassion, identifying barriers to care and available resources to create the best possible outcomes Guide members of the community to wellness and testing services of strategic partners and agencies Develop Personalized Care Plans & Community Strategies Create and contribute to personalized resource plans that connect clients with appropriate providers, specialists, and community resources Collaborate closely with resource providers to monitor client progress and adjust plans as needed Provide recommendations that facilitate optimal outcomes for each individual's circumstances Devise and manage action plans to host and/or facilitate events and activities which lead to community engagement and awareness of available resources to support physical and mental well-being, including but not limited to health fairs, vaccination clinics, popup events, etc. Build Community Relationships & Partnerships Build relationships with key partners and constituents to identify opportunities to educate individuals and communities on health awareness, so that they are educated on the key metrics that impact their health and well-being Identify opportunities to partner with organizations with missions and/or targeted programs similar to the KGCDC, including faith-based organizations, government agencies, and corporations to broaden the KGCDC's reach to support the community Develop strategies to increase community engagement with available health and wellness resources Communication & Program Management Manage the communication strategy related to driving health awareness throughout the community, including social media and advertising Develop relevant metrics and reports to evaluate the impact of programs and initiatives on the community and its use of those programs and initiatives Identify and attend requisite trainings, conferences, etc. to stay abreast of opportunities to advance health awareness Maintain Professional Standards Uphold the highest ethical and legal standards regarding client confidentiality Ensure compliance with all organizational, local, state, federal, and third-party payer policies and regulations Document all client interactions and progress with precision and care Qualifications Required Bilingual Spanish & English Bachelor's degree in Public Health, Healthcare Administration, Social Work, or equivalent education and experience Experience leading large-scale events for community-based programs Experience working collaboratively with diverse internal and external partners Demonstrated ability to work autonomously in fast-paced environments Exceptional communication skills Ability to analyze and interpret data to draw conclusions and communicate to constituents Proficient in the use of Microsoft Office Suite (i.e. Word, Excel, PowerPoint, Outlook) Preferred Previous experience working in a community-based non-profit environment Experience in case management or care coordination Knowledge of health and social services in the Washington DC metropolitan area Working Environment Hybrid work environment (office, community, and home); Tuesday through Saturday Alternate work schedules may be required to support Sunday activities Position requires travel within the community for client visits and events Some evening and weekend work may be required for community events This job description outlines the primary elements of the role and is not intended to be an exhaustive list of responsibilities.
