Position Summary The Community Supports Manager oversees the development, implementation, and quality of non-licensed programs at Heart to Heart Home Care. This role ensures services meet the needs of individuals with developmental and intellectual disabilities, comply with state and federal guidelines, and align with our mission. The Manager works closely with internal teams and external partners to maintain program excellence, foster community connections, and ensure consumer satisfaction. Responsibilities include using Therap and HHAeXchange for documentation, billing, and compliance. This is a hybrid position (2 days remote, 3 days in-office, adjusted as needed) and requires 24/7 on-call availability.
