Position Summary The Community Supports Manager oversees the development, implementation, and quality of non-licensed programs at Heart to Heart Home Care. This role ensures services meet the needs of individuals with developmental and intellectual disabilities, comply with state and federal guidelines, and align with our mission. The Manager works closely with internal teams and external partners to maintain program excellence, foster community connections, and ensure consumer satisfaction. Responsibilities include using Therap and HHAeXchange for documentation, billing, and compliance. This is a hybrid position (2 days remote, 3 days in-office, adjusted as needed) and requires 24/7 on-call availability. Requirements Must be at least 21 years of age. Two or more years of experience working with individuals with developmental and intellectual disabilities — flexible for the right candidate; training provided if needed. Bachelor's degree preferred, but not required. Must pass state and federal background checks with no disqualifying factors. Valid NJ driver's license and reliable personal vehicle (mileage reimbursement provided). Skills & Competencies Strong written and verbal communication skills. Knowledge of best practices in program development and community supports. Knowledge of Therap and HHAeXchange preferred; training available. Proficiency in Microsoft Office and related computer systems. Strong organizational skills with the ability to manage multiple priorities. Excellent interpersonal skills to work effectively with diverse groups.
Work Environment & Travel Hybrid:
2 days remote, 3 days in-office; may be adjusted to meet program needs. Travel throughout New Jersey for program development and consumer visits. Evening and weekend hours as needed. 24/7 on-call availability required for urgent situations.
