Company Overview:
Communities For People (CFP) serves youth and families through community-based residential programs, managed care networks, home based services, adoption, foster care and outpatient services. CFP strives to achieve permanency, independence and integration into the community for the youth and families we serve. CFP focuses on strengths to foster self, family and community empowerment. EOE Treatment Coordinator/Clinician sign on and retention bonus: $500 Bonus at 90 days for Qualified candidates after successful completion of the introductory period.
Additional retention bonus on qualification:
$500 at 180 days and an additional $1000 bonus at the year mark.
Residential Clinician Responsibilities:
Oversee the treatment planning and assist with the case management needs of youth between the ages of 13-21 Ensuring implementation of family centered practice Development and implementation of Individual Service Plans Facilitates case conferences and discharge conferences for clients Oversight of coordinating psychiatric, psychological, medical and dental services Overseeing timely notification of referral dispositions Assisting in the training program of staff Actively participating in staff meeting and group supervision Providing clinical support to program staff Crisis management Participating on-call rotation
Residential Clinician Qualifications:
Must have a Master of Social Work degree or related degree At least 1 year of clinical experience preferred Must have valid driver's license and transportation Excellent writing skills / computer literacy Excellent organization and time management skills
Treatment Coordinator/ Clinician Benefits:
