Role:
The Community Support Provider plays a vital role in enhancing the quality of life for individuals within the community by offering personalized assistance and support services. This position focuses on fostering independence, promoting social inclusion, and ensuring access to necessary resources for community members. The role requires building trusting relationships with clients, understanding their unique needs, and collaborating with multidisciplinary teams to develop and implement effective support plans. The Community Support Provider acts as an advocate, helping individuals navigate social services, healthcare, and other community resources. Ultimately, this position contributes to creating a supportive and empowering environment that enables individuals to thrive within their communities.
Minimum Qualifications:
High school diploma or equivalent. Experience working in community support, social services, or a related field. Valid driver's license. Ability to pass background checks and comply with state and federal regulations.
Preferred Qualifications:
Experience working with diverse populations, including individuals with disabilities or mental health challenges. Knowledge of local community resources and social service systems. Bilingual English/Spanish.
Responsibilities:
Provide direct support and assistance to individuals in their homes and community settings to promote independence and well-being. Develop and implement individualized support plans in collaboration with clients, families, and healthcare professionals. Assist clients with daily living activities, including transportation, appointment coordination, and accessing community resources. Monitor client progress and document services provided, ensuring compliance with organizational policies and regulatory requirements. Advocate on behalf of clients to connect them with social services, healthcare providers, and other community programs. Facilitate social and recreational activities to encourage community engagement and reduce social isolation. Maintain clear and professional communication with clients, families, and team members to ensure continuity of care.
Skills:
The Community Support Provider utilizes strong communication skills daily to build rapport and effectively understand client needs. Problem-solving and critical thinking are essential for developing tailored support plans and addressing challenges as they arise. Organizational skills are used to manage schedules, document interactions, and coordinate with multiple stakeholders. Empathy and cultural sensitivity enable the provider to connect with clients from diverse backgrounds and foster trust. Additionally, knowledge of community resources and advocacy skills empower the provider to guide clients toward appropriate services and opportunities for growth.
