This role will lead the planning, implementation, evaluation, and sustainability of grant-funded programs focused on improving healthcare access, care coordination, health outcomes, and service delivery in underserved and rural communities. The ideal candidate will possess strong program management, stakeholder engagement, contract oversight, grant administration, and data-driven decision-making experience. This position requires collaboration with healthcare providers, community organizations, government agencies, technology partners, and multidisciplinary teams to ensure successful program implementation and measurable impact. Key Responsibilities Lead the strategic planning, implementation, and oversight of complex public health programs and initiatives. Develop program goals, objectives, key performance indicators (KPIs), and success metrics. Manage federal, state, and grant-funded program requirements, ensuring compliance with all applicable regulations and reporting standards. Oversee contracts, scopes of work, budgets, expenditures, procurement activities, and deliverables. Monitor program performance through data analysis, outcome measurement, evaluation activities, and reporting. Collaborate with internal teams, healthcare organizations, community partners, and technology vendors to support program implementation. Provide technical assistance, training, and ongoing support to funded partners and stakeholders. Develop sustainability strategies to maintain program impact beyond grant funding periods. Conduct program evaluations and translate findings into actionable recommendations. Facilitate stakeholder meetings, site visits, presentations, and community engagement activities. Prepare quarterly, annual, and ad hoc reports for leadership, funding agencies, and external stakeholders. Support implementation of healthcare technology solutions, care coordination initiatives, and service delivery improvements. Promote collaboration across public health, healthcare, and community sectors to improve health outcomes and access to care. Ensure program activities remain audit-ready and compliant with funding requirements. Required Qualifications Bachelor's degree in Public Health, Health Administration, Healthcare Management, Nursing, Social Work, Public Administration, Business Administration, or a related field. Five (5) or more years of progressively responsible professional experience in public health program management, healthcare program administration, grant management, or related fields. Experience managing contracts, budgets, grants, and compliance requirements. Demonstrated experience leading complex, multi-partner initiatives or statewide programs. Strong project management, organizational, and problem-solving skills. Experience utilizing data to drive program planning, evaluation, and continuous improvement. Excellent written, verbal, presentation, and stakeholder engagement skills. Ability to manage multiple priorities in a fast-paced environment. Preferred Qualifications Master's degree in Public Health (MPH), Healthcare Administration (MHA), Public Administration (MPA), Business Administration (MBA), Nursing, or a related field. Experience with rural health, maternal and child health, population health, healthcare access initiatives, or healthcare transformation programs. Experience providing training, technical assistance, or consultation to healthcare providers and community organizations. Knowledge of healthcare technology, electronic health records, interoperability standards, telehealth, or care coordination models. Experience working with federal grants and reporting requirements.
