This role works closely with clinical staff, patients, families, and internal departments to ensure timely, appropriate staff assignments and optimized daily operations. The Scheduling Coordinator is a key team member in supporting patient access, continuity of care, and overall departmental effectiveness. Assign and schedule caregivers for clients at home or in facility settings based on availability, language, and care needs. Confirm caregiver acceptance and availability for all assigned visits. Ensure all visits for the following day are scheduled and confirmed prior to logging off. Document and maintain accurate scheduling records using systems such as HHAeXchange and Salesforce. Facilitate best caregiver-patient matches. Communicate caregiver assignments to patients and families; relay any special instructions or concerns. Respond to phone calls, voicemails, and emails in a professional and timely manner (within 24 hours). Collaborate with the clinical team to report and escalate any changes in condition or care-related concerns. Work closely with the HR/recruiting team to share caregiver availability needs and assist with coverage gaps. Monitor compliance with agency policies and payer/contractual requirements (CHHA, LTHHCP, MLTC, etc.). Notify contract case managers and schedulers of changes, missed visits, and any client concerns. Participate in on-call rotation, ensuring calls are answered and addressed on nights/weekends as assigned. Accept and staff all new referrals unless written denial authorization is obtained from DPS/Administrator.Education & Experience:
High school diploma or equivalent required; associate degree preferred. Minimum of 1-2 years of experience in scheduling within a home health or healthcare setting. Prior use of HHAeXchange or similar scheduling/EMR platform strongly preferred.Knowledge:
Understanding of home care workflows, caregiver roles, and patient needs. Knowledge of Medicaid, MLTC, and private duty home care scheduling practices. Familiarity with HIPAA, caregiver credentialing, and visit verification requirements.Skills:
Strong interpersonal communication skills - both verbal and written. Excellent customer service abilities with empathy and patience, especially when working with the elderly. Proficient in Microsoft Office (Word, Excel, Outlook) and basic office equipment. Organized, reliable, and detail-oriented with strong multitasking capabilities.Abilities:
Work independently and collaboratively in a high-pressure, fast-paced environment. Remain flexible and solutions-focused while managing dynamic schedules. Deliver excellent customer service with empathy and patience, especially when working with elderly clients. Communicate clearly, calmly, and professionally across diverse backgrounds. Adapt to new systems, protocols, and operational workflows as needed.
