Home Health Intake Coordinator / Scheduler Arlington, TX 76006 $18

Additional duties for this Scheduler role include, but are not limited to: Ensuring the correct clinicians for a patient as per the patient's Start of Care. Sending reports to show your team work daily in excel. Ensuring clinicians are completing their notes in a timely fashion. Making sure referral documents received are processed correctly and turned over the the correct department. Essential Duties and Responsibilities Receive and process patient referrals and physician orders. Verify patient information, insurance eligibility, and required documentation. Coordinate and schedule skilled nursing, therapy, and aide visits according to physician orders and staff availability. Maintain accurate scheduling records and update patient information in the EMR system. Communicate effectively with clinicians regarding schedule changes, patient needs, and visit coordination. Answer incoming calls and assist patients, families, and referral sources professionally. Monitor staffing coverage to ensure timely patient visits and continuity of care. Follow up on pending documents, authorizations, and physician signatures. Ensure compliance with agency policies, HIPAA regulations, and state/federal home health guidelines. Assist with administrative tasks and office coordination as needed. Hiring Requirements Education High school diploma or equivalent required. Associate degree or healthcare-related education preferred. Experience Minimum of 1-2 years of scheduling, intake, or administrative experience in a healthcare or home health setting preferred. Experience with EMR/EHR systems and scheduling software preferred. Skills and Qualifications Strong organizational and multitasking abilities. Excellent communication and customer service skills. Ability to work in a fast-paced environment and manage changing priorities. Knowledge of medical terminology and home health operations preferred. Proficient in Microsoft Office (Word, Excel, Outlook). Ability to maintain confidentiality and professionalism. Strong attention to detail and problem-solving skills. Physical Requirements Ability to sit for extended periods and work on a computer. Ability to communicate effectively by phone and email. Work Schedule Full-time / Part-time (depending on agency needs) May require occasional overtime or weekend coverage. Preferred Qualities Team-oriented and dependable. Compassionate and patient-focused. Ability to handle sensitive situations professionally. Strong time-management and coordination skills
