Hospice Care LVN Intake/Scheduler - Immediate Office Position Dallas, TX 75254 Full-time Full-time Novel Hospice Care has
IMMEDIATE OPENING!
!! We are looking for an LVN/LPN Intake/Scheduler with previous Hospice intake/scheduling experience to assist the Clinical Manager/Administrator. Primary functions would be to assist Hospice Clinical Manager and to perform intake/scheduling activities. Day to day activities include but not limited to: Intake and scheduling of patients visits Handle setting up DME needs of patients Coordinating care with the Field staff as needed Coordinating IDT meeting - preparing documents as needed QA patient Charts for completeness Other intake/scheduling related tasks as required Knowledge of Kinnser/Wellsky EMR required Knowledge of Excel, MS Word and Google Docs is a plus!!! Bilingual (English/Spanish) is a plus Kinnser/Wellsky Experience is a plus
