Job Description

We are a home healthcare agency that has been in business for over twenty years. We provide care management and home healthcare services to geriatric clients. We are currently seeking a seasoned and experienced full-time, in-office Home care scheduling Coordinator. The ideal team member must be able to manage multiple priorities (phone calls and many client/caregiver schedules) simultaneously to ensure all clients are appropriately serviced. This is a high-volume role.
Principle Responsibilities:
Take full ownership of assigned cases, ensuring comprehensive coverage aligned with clients' care schedules. Collaborate closely with caregivers and the care team to maintain high-quality care and deliver services promptly. Facilitate the matchmaking process between clients and caregivers, ensuring a harmonious long-term partnership and consistent scheduling. Manage, update, and oversee the scheduling system to optimize efficiency and accuracy. Thoroughly document all communication and scheduling activities in Axis Care and Google Sheets, including calls and notes. Notify clients promptly of any changes to their scheduled care in accordance with their preferences. Maintain open communication with caregivers regarding any schedule adjustments or delays in relief, ensuring smooth operations. Acknowledge exceptional performance and assistance from caregivers, providing feedback to the management team. Report any concerns or issues regarding caregivers or clients to the management team for prompt resolution. Ensure employee compliance in accordance with the company policies, ethics standards, and values Direct caregivers to the appropriate staff member for non-scheduling related matters, facilitating efficient problem-solving. Provide weekly reports summarizing key activities and developments Participate in on-call scheduling, which will include every other weekend calls from caregivers and clients Alternating Weekend On-Call Scheduling Responsibilities Take full ownership of assigned cases, ensuring comprehensive coverage aligned with clients' care schedules. Hours:
In the office: Monday- Friday 9:00AM-5:00PM. We are located on the SW edge of the Galleria Area, 77057 On-call remote scheduling will be every other weekend, starting at 5:00PM on Friday and ending Monday at 9:00AM. During remote on-call hours, you can maneuver throughout your daily life with the assurance that you are able to answer calls with 100% ownership in filling shifts
Qualifications:
Proven track record in a high-volume, home healthcare scheduling position (must have 4+ years of proven experience) Exceptional customer service and phone etiquette Ability to prioritize tasks/projects while meeting company goals Proactive and independent with the ability to take initiative and work independently Prolonged periods of sitting at a desk, working on a computer, and utilizing a headset to complete multiple calls Ability to work in a fast-paced environment and work well under pressure Ability to build relationships with caregivers, clients, and company team members Must be proficient with Google Docs, Microsoft Word & Excel Excellent communication, customer service, and problem-solving skills Self-starter with exceptional organizational skills and the ability to work in a fast-paced environment. Reliable internet connection while working remotely every other weekend. Ability to read, write, and effectively communicate in English Ability to pass a state/federal criminal background check Educated and possess a full understanding of HIPAA regulations; maintaining strict confidentiality of patient and client information
