Unlike a traditional medical office, our providers travel to patients in their homes, assisted living facilities, and other care settings. This role is critical to ensuring patients are seen on schedule and providers maintain efficient daily routes. The ideal candidate is a strong communicator who thrives in a fast-paced environment, can think quickly under pressure, and is comfortable managing multiple moving parts throughout the day. You must be proficient with technology, Excel, and EHR systems, while demonstrating strong follow-through and problem-solving skills. This is not a front-desk clinic scheduling role. Our providers travel to patient homes and facilities throughout the community, requiring constant coordination with patients, caregivers, facilities, and home health agencies. Duties Schedule patient visits efficiently across multiple providers and geographic territories. Confirm upcoming patient appointments via phone calls, text messages, and other communication methods. Follow up with patients who do not respond and coordinate with family members, caregivers, facilities, and home health agencies as needed. Maintain accurate scheduling information and patient records within the Electronic Health Record (EHR) system. Coordinate urgent, same-day, and rescheduled visits while minimizing disruptions to provider schedules. Monitor provider schedules and proactively identify scheduling gaps, route inefficiencies, or potential conflicts. Manage appointment cancellations, rescheduling requests, and follow-up reminders to reduce missed visits. Verify patient demographics, contact information, insurance information, and visit details. Communicate professionally with patients, providers, home health agencies, facility staff, and internal team members. Assist with administrative tasks including documentation, data entry, reporting, and record maintenance. Escalate concerns and scheduling barriers promptly to ensure continuity of patient care. Qualifications Previous experience in medical scheduling, healthcare administration, patient coordination, or a related role. Experience working within an Electronic Health Record (EHR) system. Proficiency in Microsoft Excel and Microsoft Office Suite. Excellent verbal and written communication skills. Strong organizational and multitasking abilities. Ability to work independently and make decisions in a fast-paced environment. High attention to detail and accuracy. Comfortable learning and utilizing new technology platforms and software. Ability to remain calm, professional, and solution-focused under pressure. Demonstrates initiative, accountability, and strong follow-through. Preferred Qualifications Home Health experience strongly preferred. Experience coordinating care with home health agencies, assisted living facilities, skilled nursing facilities, or other post-acute care settings. Experience in wound care, mobile healthcare, hospice, home-based primary care, or related healthcare services. Knowledge of provider scheduling and route planning. Why Join Us?
