245D Designated Manager (Designated Coordinator Position)

245D Designated Manager or Designated Coordinator Position Job Summary We are seeking a compassionate and dedicated individual to perform 245D program management and oversight for caregivers/vulnerable adults in Big Lake program location. Summary Oversight of the programs' responsibilities assigned in the person's support plan/addendum. Facilitate the accomplishment of outcomes related to person-centered planning/service dely. Instructions and assistance to direct support staff implementing the support plan, outcomes, including direct observation of service delivery to assess staff competency, along with evaluating effectiveness of service delivery. Ensure incident/emergency training/reporting/review procedures are followed Ensure staff competency, including requirements for staff orientation and on-going training. Maintain office duties at the direction of license holder/supervisor Experience Previous 245D program experience. Full-time work experience providing direct care services to persons' with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children. Strong documentation, attention to details & communication skills are essential.
