BOE QUALIFICATIONS 1.
High School Diploma or GED coupled with a minimum of one year experience in coordinating a program, with preferred experience working in the health care field and/or geriatrics setting required. 2. Computer literacy and good data entry skills are required. 3. Electronic Health Records experience (i.e. NextGen) preferred. 4. Bilingual Spanish is highly preferred Essential Job Duties 1. Receives incoming pick-up calls; makes outgoing calls to clients to alert them of driver's arrival time, and maintains daily list of cancellations, noting reasons for cancellation. 2. Coordinates and schedules AltaMed PACE participants for transportation services (from scheduled destinations). 3. Ensures a smooth delivery of transportation service by problem solving and coordinating with AltaMed Lead drivers and/or contracted vendor designee. 4. Coordinates with PACE staff for transporting of specialized medical appointments to and from participants' home or PACE center, and to various medical sites. 5. Performs data entry of participants' days of attendance, including specialist appointments. 6. Coordinates and confirms driver routes (master driver manifest) to maximize vehicle capacity and participant satisfaction. 7. Assists and communicates updated PACE participant days of attendance and demographic information with AltaMed Data Entry Department and/or Altamed Transportation. 8. Daily communication with transportation drivers to ensure they are informed of participants' individual transportation needs/or concerns (i.e. walker, wheelchair, cognitive status, etc.). 9. Monitors driver's daily logs to ensure compliance; performs daily collecting, logging and tallying, as needed, of all trips provided by AltaMed transportation (i.e. cross reference monthly invoices with number of trips). 10. Participate in daily PACE Interdisciplinary Team Meeting as well as appropriate meetings/committees as required. 11. Responds to any inquiries by drivers concerning patient/participant pick-ups, with timely and efficient response. 12. Reports all obstacles or difficulties to Department Supervisor concerning the safe, efficient, and courteous provision of transportation services provided to PACE participants. 13. Participate in daily PACE Interdisciplinary Team Meeting as well as appropriate meetings/committees as required. 14. Perform all other related duties as assigned.
