At Apple Valley Village Health Care Center, you'll be part of a supportive, mission-driven team that takes pride in making a difference every day for residents and families alike. As a Healthcare Appointment/Transportation Coordinator at Apple Valley Village Health Care Center , you will schedule appointments/ telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC. Our ideal candidate has prior experience working in healthcare and/or patient scheduling. This person is organized, detail-oriented, and has excellent communication skills.

Friday Shift Available:

8:00 AM•4:30

PM Wage Range:

$24.00•$26.00 / hour depending on experience

Healthcare Appointment/ Transportation Coordinator Responsibilities:

Schedule appointments/telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC. Provide excellent customer service to residents, family members and community providers. Assist with uploading documents into EHR. Fax census activity report to pharmacy. Complete discharge census and discharge summary observations. Perform additional tasks as needed.

Healthcare Appointment/ Transportation Coordinator Qualifications:

Prior experience in healthcare and/or patient scheduling preferred. Must possess excellent customer service and communication skills. Ability to work independently and meet deadlines. Must be very detail oriented and able to work with multiple interruptions. Working knowledge of pertinent State and Federal regulations.
