$20-$25/hour DOE Position Summary Ascend Home HealthHospice is seeking a compassionate, organized, and relationship-focused Bridge Palliative Care Coordinator to support the growth and coordination of our palliative care program. This role serves as a key connection between patients, families, providers, referral sources, and the interdisciplinary care team to ensure patients receive exceptional supportive care focused on quality of life and symptom management. The ideal candidate is highly motivated, professional, detail-oriented, and passionate about serving patients with serious illness while helping guide them through available care options and resources. Responsibilities Coordinate and support daily operations of the Bridge Palliative Care program Communicate with referral sources, providers, patients, and families regarding services and eligibility Assist with scheduling patient visits and provider coordination Track referrals, admissions, and patient follow-up needs Maintain accurate documentation and communication within the EMR system Support patient and family education regarding palliative care services Collaborate with clinical and administrative leadership to improve workflow and patient experience Build and maintain positive relationships with community partners and referral sources Participate in program development and growth initiatives Assist with care coordination and transitions between care settings when appropriate Qualifications Experience in healthcare coordination, medical office operations, hospice, home health, or palliative care preferred Strong communication and customer service skills Excellent organizational and multitasking abilities Ability to work independently and collaboratively within a team environment Comfortable working with patients and families facing serious illness EMR experience preferred Knowledge of healthcare terminology and insurance processes is beneficial What We Offer Competitive wage based on experience Meaningful work that impacts patients and families Supportive and collaborative team environment Opportunity to help grow an innovative palliative care program Professional growth and leadership opportunities At Ascend Home HealthHospice, our mission is to Serve, Lift, and Unite through compassionate care and meaningful relationships.
