Open to candidates with strong chronic care experience (e.g., primary care, internal medicine) who are interested in transitioning into cardiology. This role focuses on managing patients with chronic cardiovascular conditions such as hypertension, heart failure, and related comorbidities, with an emphasis on longitudinal care, medication optimization, and proactive disease management.
