Care Facilitator

• Job Summary:

• The Care Facilitator is responsible for assisting the Consumer/Member in the approval process for the Private Services division of the Agency.

She/he is knowledgeable of Medicaid program regulations.

• Duties and Responsibilities:

• May serve as an interpretive liaison between Third Party Assessors and Consumer/Member per Consumers/Members request.
• Perform scheduled supervisory visits to Consumer/Member, in their home, on a monthly basis per State Program guidelines.
• Assist with internal and external transfer process.
• Responsible for accurately documenting changes in Consumer/Member information.
• Produce Incident Reports when needed.
• Verify Consumer/Member eligibility and authorizations prior to admission and thereafter as directed by State Program Guidelines.
• Participate in after hours and weekend On-Call rotation.
• Prepare for State and Agency audits.
• Requirements
• :• At least a high school diploma.
• A minimum of 1 year of experience in working in an office environment, is preferred.
• Ability to provide proof of right to work in the United States.
• Proficiency in Microsoft office including but not limited to Excel and Microsoft word.
• Must have a current driver's license and clean driving record to get on company insurance policy.
• Willing to travel as deemed necessary.
• Ability to pass a criminal background check.
