Develop and implement personalized care plans that cater to the unique needs and goals of each client Conduct initial assessments, ongoing evaluations, and ongoing care planning to ensure the highest level of care and support Collaborate with healthcare providers, family members, and other care team members to ensure seamless care coordination Monitor and report on client progress, identifying areas for improvement and implementing changes as needed Provide education and support to clients and their caregivers on disease management, medication, and overall health Manage and supervise care teams, ensuring that all team members are equipped to provide the highest level of care Maintain accurate and timely records of client care and services Identify and address potential issues or concerns, escalating them to management as needed About the
