Job Description
•Client Care Plan• We are looking for someone extremely reliable to take care of a client Monday-Friday 11AM-7PM. This client will be located in Erin, TN. The care will be for a gentleman with Huntington's Disease. We are there to support his wife. She mostly needs us there to help out with her husband if she needs to run errands or get out of the house. She has a child that participates in after school activities, so her main need is someone being there for the client while she takes her children to after school activities. FirstLight Home Care is a company based out of Clarksville with primarily team members from Clarksville, so reliability is of the uptmost importance. We will need someone extremely driven to work consistently M-F, as we do not have many team members from Clarksville willing to drive to Erin, TN to work a call out scenario. >. This role offers an excellent opportunity to make a meaningful difference in the lives of seniors and individuals with disabilities by providing exceptional personal care and support. The ideal candidate will possess a strong background in patient care, with experience in assisted living, nursing homes, or home health environments. Responsibilities Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Observe and monitor patients' conditions, documenting vital signs and medical observations accurately using Electronic Health Record (EHR) systems. Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA regulations. Support patients with dementia or Alzheimer's disease through specialized memory care techniques and behavioral management strategies. Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. Prepare nutritious meals tailored to individual dietary needs and assist with light housekeeping tasks including cleaning and laundry to ensure a safe environment. Collaborate with healthcare professionals and family members to coordinate care plans and update medical documentation regularly. Skills Strong knowledge of medical terminology, vital signs monitoring, and patient care procedures. Experience working with individuals with developmental disabilities or in hospice and dementia care settings. Proficiency in EMR/EHR systems for accurate documentation of patient information. Ability to perform caregiving duties such as assisting with ADLs, medication administration, and catheter care confidently. Excellent observational skills for patient safety and condition monitoring. Familiarity with home & community care practices, long-term care environments, and assisted living facilities. CPR certification is required; additional certifications such as hospice care or dementia training are advantageous.
