In this vital role, you will provide high-quality patient care across various settings, including assisted living, nursing homes, and home health environments. Your expertise will help enhance the well-being and independence of individuals with diverse needs, including those with developmental disabilities, dementia, or undergoing hospice care. This position offers an opportunity to make a meaningful difference in the lives of your patients while working in a supportive and energetic environment. All roles are paid positions, emphasizing the value we place on your skills and dedication. Responsibilities Monitor and observe patients closely to identify changes in condition and report findings accurately using electronic health record (EHR) or electronic medical record (EMR) systems. Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Administer medications and manage medication records following HIPAA guidelines to ensure patient safety and confidentiality. Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to monitor health status regularly. Support patients with specialized care needs including Alzheimer's disease, dementia, memory care, hospice support, catheter care, and medication administration. Assist with meal preparation, feeding, cleaning, laundry, and maintaining a clean environment to promote comfort and hygiene. Document medical records accurately while adhering to medical terminology standards and privacy regulations. Use EMR/EHR systems efficiently for documentation of patient care activities and medical updates. Collaborate with healthcare teams to develop personalized care plans tailored to each individual's needs in home or community settings. Support clients in community-based programs by helping with transportation, home safety assessments, and community engagement activities. Skills Proven experience in patient observation and providing compassionate senior or home health care. Strong knowledge of assisted living environments, long-term care settings, or nursing homes. Familiarity with EMR/EHR systems for accurate documentation of patient information. Skilled in assisting with ADLs such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts. Experience working with individuals who have developmental disabilities or cognitive impairments such as Alzheimer's or dementia. Knowledge of medical terminology relevant to caregiving tasks; CPR certification is required for emergency response readiness. Ability to perform catheter care safely and competently; experience with hospice care preferred. Competence in taking vital signs accurately and recognizing signs of distress or deterioration. Experience in medication administration following prescribed protocols while maintaining HIPAA compliance. Strong communication skills for effective collaboration with healthcare teams and families. Ability to handle cleaning duties, laundry tasks, meal preparation, and general household management when needed. Prior experience working within home & community care environments is highly valued; familiarity with medical documentation is essential.
