They advocate for individuals served to develop psychosocial recovery skills necessary to regain and maintain personal control over aspects of their lives affected by their illness.

The Case Aide Assistant provides highly valued support to the assigned treatment and is responsible for assisting the treatment team in carrying out interventions described in the Individual Plan of Service (IPOS) under the supervision of the team supervisor. They advocate for individuals served to develop psychosocial recovery skills necessary to regain and maintain personal control over aspects of their lives affected by their illness.
Compensation Range:
The starting pay for this position is $14.88 - $17.50 hourly rate, and is based on non-discriminatory factors such as skills and experience. Please see below for a summary of job responsibilities and qualifications! Support the assigned treatment team by assisting with administrative tasks and documentation. Complete and distribute letters of engagement, re-engagement, caring letters, and other documentation on behalf of the treatment team. Manage the transition protocol trackers for the assigned team(s). Print and distribute annual paperwork, IPOS, periodic reviews, letters of concern, and other applicable documentation as assigned. Assist assigned team(s) in the completion of scheduled National Outcome Measures (NOMs) and other assessments. Assist assigned team(s) in linking individuals served to eligible benefits and resources. Collaborate with the assigned treatment team(s), agency staff, and external partners including community resources to support coordination of care for individuals served. Attend team, department, and agency meetings as assigned. Utilize a trauma-informed approach to deliver services. Completes all required documentation completely, accurately, and in accordance with CNS policy. Fulfills mandatory education requirements of CNS including internal training and community education. Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements. Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff. Assume responsibility for professional growth by current reading and attendance at educational programs and conferences. Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs. Perform other related duties as assigned.
Qualifications:
Required High school diploma or equivalent Valid and unrestricted Driver's License Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements Prior experience in a customer service, administrative, and/or clerical role Proficient with computers and working knowledge of Microsoft office products. Strong customer service, communication, time management, and organization skills. Ability to work as a member of a multidisciplinary team, and community effectively, professional, and courteously. Ability to work closely with persons with mental or physical limitations. Ability to contribute to an inclusive environment that recognizes the value and contributions of all persons regardless of race, ethnicity, national origin, gender, religion, age, marital status, sexual orientation, gender identity, or disability. Ability to use discretion and judgment when handling matters of a sensitive or confidential nature. Preferred Prior work or volunteer experience in a healthcare or mental health setting, including working with adults with severe and persistent mental illnesses. Experience with electronic health record (EHR) systems.
