Job Description

Reporting to the Program Manager, the Case Coordinator, Homelessness Prevention provides supportive case management primarily focused on helping individuals/households obtain and maintain safe, stable housing. This role includes assessing housing needs and barriers, creating housing stability plans, connecting clients to community resources, and providing ongoing coaching (financial literacy/budgeting). Responsibilities also include coordinating with treatment teams/providers, ensuring follow-through appointments, conducting monthly housing inspections for safety/cleanliness and transporting clients as needed. The coordinator completes intake paperwork, maintains releases of information, conducts assessments and annual reviews, documents all case management activities, and ensures client files and recertification documentation are complete.
