Job Description

Category RN- Case Management Job Type Contract Step into a vital role supporting patient care as a registered nurse specializing in case management within an acute inpatient environment. This contract position offers the opportunity to work closely with multidisciplinary teams, ensuring that patients and families receive personalized, coordinated care. Schedule and Hours:
5x8-hour day shifts Every other weekend required
Qualifications:
Active RN license required Minimum of 2 years' recent acute inpatient case management experience is essential Strong clinical assessment, care planning, and patient advocacy skills Expertise in motivational interviewing, self-management, and critical thinking Experience coordinating complex care across the continuum, especially following critical events or new diagnoses Excellent communication abilities and a collaborative approach to care
Key Responsibilities:
Conduct thorough assessments of patient and family needs—including biomedical, psychological, social, and functional aspects—to inform recovery plans Develop and implement individualized patient-centered care plans for optimal outcomes Foster patient and family engagement, advocacy, and communication regarding health education and access to resources Utilize professional judgment to counsel on treatment decisions, care transitions, and sensitive end-of-life issues such as advanced care planning Provide seamless coordination during transitions between care settings Collaborate with care teams to address complex medical and social needs, including facilitating interdisciplinary care conferences Monitor, evaluate, and revise care plans as needed to ensure quality outcomes Participate in initiatives targeting risk reduction and quality improvement Address concerns related to abuse, neglect, or violence, intervening as appropriate If you have a passion for supporting patients through intricate healthcare journeys and thrive in a collaborative, high-impact role, submit your application today to advance your case management career!
