Bachelor's degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years of recent experience working with elders or adults with disabilities; or Bachelor's degree and two years of clinical experience in the care of elders or people with disabilities.
Responsibilities:
Conducting on-site visits with each member at the qualified setting in accordance with assigned schedules. Evaluating, supporting, and training AFC caregivers on orientation and thereafter. Completing progress notes corresponding with each on-site visit or encounter and upon significant change; Conducting the initial and ongoing evaluations of the members in a qualified setting; Provide oversight to AFC caregivers and direct care staff in conjunction with the registered nurse; Participating and assisting with the development of the member's interim and final AFC plan of care with input from the member or responsible party, all members of the MDT, and other individuals designated by the member. Ensuring implementation of the member plan of care Reviewing and documenting the fire and safety procedures for the qualified setting; Follow up on reports of incomplete Caregiver and DCA documentation weekly. Provides supportive casework designed to restore client's to their optimum level of social adjustment. This includes assisting clients and their families to understand, accept and follow medical recommendations. Effectively communicates with the Lead case manager and Program Director and provides updates on clients. Assists clients utilize the resources of their families and the community at large. Resource utilization may include referring the clients to community resources or acting as an intermediary on behalf of the clients with other health and welfare agencies. Effectively communicates with clients and family. Keeps the client informed ongoing. Maintains HIPPA with all clients, only informing staff who have the need to know to protect patient confidentiality and PHI. Communicates effectively with the Administrator, Lead case manager or program director regarding scheduled visits and changes to the schedule on a weekly basis. Attends and participates in daily case conferencing meetings to establish best practices for the individual client's needs. completing and submitting progress note corresponding with each on-site visit or encounter, or more often as the member's condition warrants at the end of every week so that all medical records are intact and up to date. Coordinates Community Services that may be available to the client to assist in safe home care needs. Reporting changes in the member's condition to the member's nurse Assisting with making referrals to appropriate service providers if the member requires services other than those provided by the provider. Conducting regular, periodic evaluations, at least annually, to ensure that each qualified setting where AFC is provided meets the requirements. Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office. Assures that visits are not done that cannot be billed. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event's occurrence. Submits written time off requests 2 weeks or more in advance of planned time off. Assist caregivers with pre-employment process to assist with application process such as gathering documents required for hire/ CPR tests etc Other duties as assigned
