Job Description

Case Manager Day Hours- 8:00am-4:00pm Monday-Friday We have an awesome bonus for new hires. $300 after 3 months of the hire date if you're still with us another $300 after 6 months of the hire date if you're still with us an extra $900 after 12 months of the hire date if you're still with us! Responsibilities:
Work with individuals on assigned caseload and their support teams to develop - and update as needed - their Person Centered Support Plan (PCSP), as well as other plans, necessary to improve independence and ensure an overall quality of life consistent with their lifestyle preferences. Manage assigned individual case records. Assure all necessary documentation is up-to-date and meets agency and state standards. Assure all deficiencies noted through Case Records reviews and Program Quality Assurance reviews are corrected immediately. Regularly monitor the quality of services provided to individuals on assigned caseload. Make home visits to residential programs and be available to individuals on assigned caseload and their family for crisis intervention and/or making referrals to community agencies as necessary. Maintain and update all documentation and information necessary for annual assessments for individuals on assigned caseload. Schedule support team meetings and other meetings as needed, advocating for individuals served on assigned caseload. Cooperate with service provider agencies in following employment, educational, habilitation and/or residential plans to support individuals served in the least possible restrictive environment. Develop and mobilize "natural support networks" such as family, church, and friends in the community. Assist individuals in accessing SSI, Medicaid and other financial benefits. Help develop locally needed services enabling individuals to receive the necessary Home and Community Based Services (HCBS). Establish links between individuals and service providers. Determine financial eligibility in accordance with Kansas Department for Aging and Disability Services (KDADS) regulations.
Experience:
The Case Manager preferably will have a Bachelor's Degree (human services field or a related discipline) and 6 months experience working with individuals with intellectual and developmental disabilities. However, experience with individuals who have intellectual or developmental disabilities may be substituted for a Bachelor's Degree at the rate of 6 months of experience for each semester required to obtain the degree. Valid Driver's License and the ability to be covered by corporate vehicle insurance. Must present a professional manner and possess working knowledge of computers. Must provide a means for emergency contact by corporation personnel.
Must have the ability to:
perform physical maneuvers such as bending, kneeling, stooping, reaching, etc.; recognize natural teaching opportunities and act upon them; be sensitive and caring regarding the personal care needs of persons served; respond to undesirable (sometimes aggressive) behaviors while maintaining calm and composure; be physically and mentally capable of operating corporate vehicles in a safe manner; report for work on site, on time, work for the full shift, and be flexible regarding work location; communicate through verbal, written and electronic mediums; observe, ask questions, listen actively, and give and accept feedback effectively; maintain a high energy level, adaptability, assertiveness, patience and ability to work in team environment.
