Clinical/Peer Services Supervisor Overview:
The Case Manager at Monarch Mental Health plays a critical role in coordinating care for clients by conducting comprehensive assessments, developing individualized care plans, and connecting clients to appropriate resources and services. This role ensures that clients receive the necessary assistance to achieve their recovery goals, while acting as a liaison between clients, peer support specialists, therapists, and external service providers. The Case Manager emphasizes care coordination and leverages peer support to facilitate task completion, promoting client progress and well-being.
Key Responsibilities:
Needs Assessment and Service Planning:
Conduct thorough Case Management Needs Assessments within 24 hours of facility intake to identify clients' needs, strengths, and challenges. Develop individualized plans in collaboration with clients, peer support specialists, and leadership outlining specific goals and the steps needed to achieve them. Integrate peer support services to assist clients with task completion and promote progress.
Resource Coordination and Advocacy:
Connect clients with internal and external resources, including housing, employment, education, healthcare, and mental health services. Advocate for clients within Monarch Mental Health and with external service providers to ensure timely and appropriate access to services. Coordinate referrals to community organizations and outside providers to support clients' ongoing needs.
Ongoing Support and Monitoring:
Regularly monitor clients' progress toward service plan goals, providing continuous support and adjusting plans as necessary. Collaborate with therapists, peer support specialists, and other clinical staff to ensure coordinated, comprehensive care for each client. Provide assistance to clients in navigating complex systems and completing necessary tasks, promoting their independence. Emphasize strong organizational skills and the ability to collaborate effectively in a team setting. Experience in connecting individuals to outside services is highly desirable.
Documentation and Compliance:
Maintain accurate and up-to-date records of client interactions, progress, and service plans in the electronic health record (EHR) system. Ensure all case management activities adhere to organizational policies and comply with local, state, and federal regulations. Participate in audits and quality assurance processes to ensure service delivery meets required standards.
Client Empowerment and Relationship Building:
Build trusting, empathetic relationships with clients, offering guidance and support throughout their recovery journey. Empower clients to take ownership of their recovery by providing them with the tools and resources they need to succeed. Foster a collaborative environment where clients feel supported in achieving their personal goals.
Qualifications:
Education and Certification:
Bachelor's degree in Social Work, Psychology, or a related field. Master's degree in Social Work or Counseling preferred. Certification in case management or mental health (e.g., CCM, CMHS) is preferred.
Experience:
Minimum of 3 years of experience in case management or care coordination in a mental health or social services setting. Experience working with individuals with severe and persistent mental illness (SPMI) preferred. Familiarity with Medicaid, private insurance billing, and resource navigation is a plus.
Skills:
Strong communication, organizational, and interpersonal skills. Ability to work effectively in a multidisciplinary team and manage multiple client cases. Proficiency in EHR systems and care coordination software.
