Full Job Description Open Positions:
Northmont School Case Manager (FT) Adult Intensive Community Based Case Manager (FT) Adult Community Based Case Manager (FT) CIT Co Responder with Englewood Police (PT) Supported Employment Community Based Case Manager (FT) General Summary The general responsibilities of the position are to provide community psychiatric support treatment interventions to clients who have multiple service needs. This includes developing mutual working treatment plans with clients, and assisting clients in reaching treatment plan goals. The Case Manager will demonstrate time management skills, ability to work independently out in the community, and the ability to work in a larger team.
Minimum Qualifications:
Experience providing case management to clients with severe and persistent mental illness and dual-diagnosed populations preferred. A Bachelor's degree in counseling, social work, human services or a related field, or an Associate's degree in human services with 3 years of experience, or an Associate's degree in a human services field.
