TITLE:
Case Manager
DEPARTMENT
Clinical/Residential Services
REPORTS TO
Director, QI & Admissions FLSA:
Exempt Position Summary This position will provide ongoing client support and expertise through comprehensive assessment, planning, implementation, and evaluation of our client needs. This individual has accountability for the care, coordination, and discharge planning for all clients. Our facility is also proud to be a leader in LGBTQ+ affirming care. LSW or LPC required. Essential Duties and Responsibilities Advocating for client during Semi-Annual Review, placement preservation meetings, and court appearances Accompanying clients to outside appointments including medical appointments, driving agency vehicle to and from locations per client needs Facilitating and conducting Community Psychiatric Support Treatment groups Assisting client in acclimating to the milieu Providing psycho-social education and support in developing independent living skills and providing emotional support to clients in need Coordination of integration of case management functions into client care and with external service providers, agencies, and healthcare facilities Consults with other community agencies/providers to identify potential resources for resolving clients' health and/or psycho-social issues Functions as a liaison to external agencies and shares information with others which may impact care of client Collaborates with clients, caregivers, physicians, and other providers to develop a comprehensive plan of care Effectively communicates care plans and other client-related activities both verbally and in writing to all treatment team members Demonstrates ability to use teaching, learning, and counseling skills to address client's needs Promotes empowerment of client and self-management of issues Lead and/or participate in interdisciplinary team meetings Utilizes skills to work independently in a variety of environments Demonstrates knowledge and skills necessary to provide services appropriate to the needs of clients based on age, gender, race, and socio-economic status Ensures completion of admission paperwork for the intake of residents Ensures completion of all required documentation for clients (i.e., Crisis Management Plans, Behavioral Intervention Plans, Personal Care Assessments, and weekly and monthly review of client progress) in a timely manner Performing other related activities as assigned Minimum Qualifications (Education, Experience, Skills) Bachelor's degree Must be at least 21 years of age. At least one-year full time equivalent paid or volunteer experience required in direct provision of care to children. Knowledge of Trauma-Informed Care and Principles, preferred experience in a Trauma-Informed Environment At least 3 years of additional work experience that can be demonstrated to be applicable to the duties listed on the job description, preferably in a residential setting. Strong interpersonal and communication skills and the ability to work effectively with a wide range of individuals in a diverse community, both internally and externally. Strong organizational skills, with the ability to meet deadlines. Ability to maintain composure in stressful situations. Must have and exercise good personal and professional boundaries at all times. Strong decision-making skills, demonstrating sound judgment. Possession of a valid motor vehicle license and willingness to operate motor vehicles in the course of employment. LPC or LSW licensure Physical Demands and Work Environment The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. The employee frequently is required to use hands or finger, handle, or feel objects, tools, or controls. The employee may occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. The employee may deal with patients who are confused, disoriented, and may be hostile and aggressive. The typical caseload is approximately 10 onsite children. The noise level in the work environment is usually moderate.

Schedule:
Monday to Friday, 8:30am to 5:30pm base schedule. Some flex time needed per client needs of caseload. Application Question(s): Are you willing to undergo a drug screen, TB test, SACWIS Search, FBI/BCI background check, and pre-employment physical?
Experience:
case management: 2 years (Preferred) working with adolescence: 2 years (Preferred)
License/Certification:
LPC or LSW (Required)
Work Location:
In-person in Cle Hts., office and laptop provided. ALL clients onsite, while possible traveling for appointments and family therapy.
