THERAPEUTIC FOSTER CARE
agency! Seraaj Family Homes, Inc. is seeking Case Managers to join our growing team based in the Huntsville area. This position will provide services to clients in Madison, Morgan, Marshall County and surrounding areas. •This position requires a Bachelor's Degree in Social Work, Psychology, Criminal Justice or a related field with a minimum of 1 year experience working with children and families .•This position is primarily responsible for (not all-inclusive): Creating and maintaining a positive rapport with clients. Providing services in the homes of the foster parents/clients and other appropriate settings by visiting clients at least 2 times per week, more depending on the client's needs Ensuring children and families receive required services per their treatment plan and individualized service plan (ISP) Interacting and physically engaging with children and youth in age-appropriate physical activities aligned with their treatment goals Assessing and advocating for options and services to meet the client's needs Educating the client and his/her treatment team on the programs and options available to meet the client's needs. Assisting in transporting the client to their appointments Connecting clients to community resources based on their individual or family needs Accurately and timely entering a summary of provided services (session notes) in the web-based system, per policy Being accessible to children, families, and internal & external customers when needed and available to report on-site in the event of a crisis Performing other assigned duties What Do We Need From You? Applicants must be highly motivated, energetic, and optimistic. Applicants must have excellent interpersonal skills. Applicants must have excellent organizational skills. Applicants must be a team player. Applicants must be tech-savvy and fluent with Microsoft Excel, Word, Outlook, and OneDrive. Employees are provided an iPad. Applicants must be trustworthy to perform duties timely in a 100% community-based working environment (no hand-holding).
