Southern Highlands Community Mental Health Center (SHCMHC) Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator II - School-Based to support youth and families within a school setting. This role focuses on coordinating care, providing support, and helping students succeed both emotionally and academically. Key Responsibilities Coordinate behavioral health, medical, and social services Assist with treatment planning under a licensed clinician Provide individual and group social-emotional learning (SEL) sessions Support students and families in crisis situations Collaborate with school staff, families, and providers Monitor progress and maintain documentation in the EHR Qualifications Bachelor's or Master's degree in a related field (Social Work, Counseling, Psychology, etc.) Valid driver's license Strong communication and organizational skills Preferred School-based or youth behavioral health experience Familiarity with CCBHC Licensure (LSW, RN, or similar)
