35 hours Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and organized Case Worker to provide social services support and case management assistance to individuals and families in need of public assistance, medical support services, institutional care, and protective services. The role involves assessing client needs, developing individualized service plans, determining eligibility for support programs, and coordinating access to community resources to promote positive client outcomes. Key Responsibilities Case Management & Social Services Provide support to individuals and families accessing public assistance and community resources Conduct comprehensive client assessments to identify social, economic, medical, and environmental needs Develop, implement, and monitor individualized service plans Determine eligibility for public assistance, food assistance, medical assistance, and related programs Connect clients with appropriate community resources and support services Client Support & Counseling Provide guidance, advocacy, and supportive counseling to clients and families Assist clients in navigating social service and healthcare systems Conduct follow-up and investigatory activities related to case management services Maintain regular communication with clients, families, and external agencies Documentation & Compliance Maintain accurate and timely case records and documentation Ensure compliance with agency policies, procedures, and regulatory requirements Monitor case progress and update service plans as needed Participate in interdisciplinary meetings and case reviewsRequired Qualifications Education:
Bachelor's Degree from an accredited college or university (required)Experience:
Minimum of 3 years of related case work or social services experience (required)Language:
