Work Setting:
Field-based (Home visits within assigned county-Northumberland County, PA) Benefits $0.70/mile travel reimbursement Weekly pay (salary + gas reimbursement) Fixed interview and joining timelines Job Overview We are looking for Enrollment Case Managers (ECM) to support HCBS (Home and Community-Based Services) programs in Northumberland County. In this role, you will: Work directly with applicants in their homes Assist individuals through the eligibility and enrollment process Act as a facilitator, advocate, and guide throughout the application lifecycle
Note:
This is a field-based role requiring extensive travel within the county. Travel Requirement Travel required throughout Northumberland County and nearby counties Must have own reliable vehicle Public transportation is not allowed Key Responsibilities Client Support & Intake Conduct in-home intake visits for HCBS applicants Explain program benefits, rights, and responsibilities Support applicants from initial identification through enrollment or termination Application Assistance Assist with application completion and submission (Compass system) Perform data entry and documentation Care Coordination Help applicants choose: Managed Care Organizations (MCOs) Life Providers Support Coordinators Ensure consumer choice and control Stakeholder Engagement Build relationships with: Healthcare providers County offices Advocacy groups Community organizations Case Management Monitor application progress and resolve delays/stalled cases Attend scheduled meetings and provide updates on availability Additional Details
