We provide care at our state-of-the-art center and in participants' homes. Serving the community since 2019, PACE North is expanding rapidly!

What You'll Do:

As a vital part of our home care team, you'll provide compassionate care to our participants in their homes. We have participants living within a 7 county area with Grand Traverse at the center.

Your responsibilities will include:

Assisting with daily living activities in the home such as bathing, dressing, cleaning, shopping and meal support. Taking vital signs and documenting participant care. Supporting our interdisciplinary team to ensure high-quality care.
