Care Team Role - Certified Nursing Assistant (CNA)
Supervisor:
Director of Nursing Job Purpose:
The CNA provides personal care and related services in the patient's residence. They function under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor.Qualifications:
Successful completion of a CNA training and presentation of a copy of the certificate of completion CNA certification or certificate issued by the state in which providing care/services, if applicable. Successfully demonstrates competency in skills required by the agency. Graduated from high school or equivalent (GED). Must be 18 years old or older. One year of healthcare experience preferred.Compensation:
PRN or Part-Time $18 - $22/Hourly Based onExperience Responsibilities:
Tasks performed by a CNA must be assigned by and performed under the supervision of an RN who will be responsible for the patient care provided by the CNA. Duties of the CNA may include: Performance of all personal care activities contained in a written assignment by a licensed health professional employee or contractor of the Agency and which include assisting the patient with personal hygiene, ambulation, eating, dressing, shaving, physical transfer, and other duties as assigned. Maintenance of a clean, safe and healthy environment, which may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the patient's dishes or laundry, and such tasks to maintain cleanliness and safety for the patient or client. Other activities as taught by a licensed health professional employee or contractor of the Agency for a specific patient and are restricted to the following: Assisting with reinforcement of dressing Assisting with tasks associated with elimination: ostomy bags, or changing bags that do not adhere to the skin Assisting with use of devices for aid to daily living, such as a wheelchair/walker Assisting with prescribed range of motion exercises Assisting with prescribed ice cap or collar Doing simple urine tests for sugar, acetone or albumin Measuring and preparing special diets Measuring intake and output of fluids Measuring temperature, pulse, respiration or blood pressure Keeping records of personal health care activities. Observing appearance and gross behavioral changes in the patient, reporting to the RN. Supervision of self-administered medication in the home is limited to the following: Obtaining the medication container from the storage area for the patient Ensuring that the medication is prescribed for the patient Reminding the patient that it is time to take the medication as prescribed Observing the patient self-administering the medication Participation in the Agency QA Program To follow the POC for each patient and timely documentation of care/services provided. Helps the patient to maintain good personal hygiene Attempts to promote patient's mental alertness through involvement in activities Gives simple emotional and psychological support to the patient and other members of the household Establishes a relationship with patient and family which transmits trust and confidentiality Reports any change in the patient's status or home situation to her immediate supervisor, the staff nurse, or the aide supervisor Works with personnel of other community agencies involved in the patient's care as directed by the staff nurse Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the professional nurse Attends in-service as required by state and federal regulations Notifying the Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with the Agency. Wears the Agency's name badge at all time they are seeing Agency patients. The Aide may also provide the following assistance with self-administered medication, as needed by the patient, in accordance with FL Section 400.488, F.S. once Med Aide training is completed: Prepare necessary items such as juice, water, cups, or spoons to assist the patient in the self-administration of medication; Open and close the medication container or tear the foil of prepackaged medications; Assist the resident in the self-administration process. Examples of such assistance include the steadying of the arm, hand, or other parts of the patient's body so as to allow the self-administration of medication; Assist the patient by placing unused doses of solid medication back into the medication container.Physical Demands:

The employee is regularly required to stand, walk, and sit, as well as talk and hear. The employee is required to use hands to operate vehicles and office equipment. The employee must occasionally lift and/or move up to 30 pounds.
