Job Description

Overview Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance with daily activities, monitor patient health, and support overall well-being in various settings such as assisted living facilities, nursing homes, or home care environments. This position offers a rewarding opportunity to work closely with individuals of all ages and abilities, including those with developmental disabilities, dementia, or recovering from illness. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor and document patient vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems. Observe patient conditions closely, noting any changes in behavior or health status related to dementia, Alzheimer's disease, or other medical conditions. Support medication administration following proper protocols while maintaining strict compliance with HIPAA regulations to protect patient privacy. Provide specialized care for memory and hospice patients, including assistance with catheter care and managing symptoms associated with chronic illnesses. Maintain cleanliness of the client's environment through light housekeeping tasks such as laundry and general cleaning to promote hygiene and comfort. Document medical records thoroughly and accurately in electronic systems to ensure seamless communication among healthcare providers. Assist with meal preparation and nutritional needs tailored to individual dietary requirements. Use EMR (Electronic Medical Records) systems effectively for documentation and communication purposes. Collaborate with nursing staff and other healthcare professionals to deliver comprehensive patient care in home health or long-term care settings. Skills Strong ability to observe patients carefully and report findings promptly. Experience working in assisted living facilities, nursing homes, or home & community care environments. Knowledge of medical terminology relevant to patient care scenarios. Proficiency in using EMR/EHR systems for documentation and record keeping. Skilled in assisting with ADLs, including transfer techniques like Hoyer lifts and catheter care procedures. Familiarity with dementia care strategies, Alzheimer's care techniques, and memory care best practices. CPR certification along with knowledge of hospice care principles is highly desirable. Ability to work effectively with individuals who have developmental disabilities or other special needs. Experience administering medications safely following prescribed protocols. Capable of handling medical documentation accurately while maintaining confidentiality under HIPAA guidelines. Competent in vital signs measurement and basic patient assessment skills. Prior experience in long-term care or home health services is preferred but not required; training will be provided if needed.
