Certified Nursing Assistant/Home Health Aide

CNA - Home Visits (PACE Program) Program of All-Inclusive Care for the Elderly (PACE) is seeking a compassionate Certified Nursing Assistant (CNA) to provide in-home care to elderly participants.

Responsibilities:

Assist participants in their homes with bathing, dressing, grooming, toileting, and mobility Support meal preparation, feeding, and light housekeeping related to care Take and document vital signs, report changes to the care team Follow individualized care plans and safety protocols Build positive, respectful relationships with participants and caregivers

Requirements:

Current CNA certification (state-approved) Valid driver's license & reliable transportation Ability to work independently in participant homes Experience with seniors or home care preferred Why Join PACE? We provide home health aide visits to our participants in multiple counties. Our territory covers the following counties; Newaygo, Montcalm, Mecosta, Lake, Mason, and Oceana.
